Background

Pack-years express the cumulative lifetime burden of cigarette smoking as a single standardised number, allowing very different smoking patterns to be compared on one scale. One pack-year equals smoking 20 cigarettes (one standard pack) per day for one year. It is calculated as cigarettes per day divided by 20, multiplied by the number of years smoked. The measure is widely used to define eligibility for lung-cancer screening and to gauge chronic obstructive pulmonary disease (COPD) and cardiovascular risk.

Interpreting the result

The value rises in proportion to both intensity and duration of smoking, so 20 cigarettes a day for 20 years and 40 a day for 10 years both give 20 pack-years. A burden under roughly 20 pack-years usually falls below common lung-screening thresholds, while 20–40 pack-years is the range at which most screening programmes become relevant. Above 40 pack-years represents heavy cumulative exposure with substantially elevated lung-cancer and COPD risk. The number quantifies exposure rather than current risk, so it should be combined with age and time since quitting.

Worked example

A patient who smoked 15 cigarettes a day for 30 years has (15 ÷ 20) × 30 = 22.5 pack-years. This sits within the screening-relevant band and, depending on age and quit status, may make them eligible for low-dose CT lung-cancer screening.

  • Garbled or uncertain self-reported smoking histories make the figure approximate; round sensibly and document the assumptions used.

  • Non-cigarette tobacco (roll-ups, cigars, pipes) does not map neatly onto a 20-per-pack convention, so estimates for these are less precise.
  • A low pack-year count does not mean zero risk; some lung cancers occur in light or never-smokers.

How do I calculate pack-years?

Divide the number of cigarettes smoked per day by 20 (a standard pack), then multiply by the number of years smoked. For example, 10 a day for 40 years is (10 ÷ 20) × 40 = 20 pack-years.

Why is 20 pack-years important?

Many lung-cancer screening programmes use a threshold of around 20 pack-years, combined with age, to define who is offered low-dose CT screening. It is a marker of sufficient cumulative exposure to justify screening.

Do pack-years account for stopping smoking?

No. Pack-years measure total past exposure only. Time since quitting is a separate factor that reduces risk and is considered alongside the pack-year figure.
